Your service location list will also state whether the service location is in a specific location or in a variety of locations depending on the case.

Once your application is approved, your case will be assigned an Intake Worker. The intake worker will visit your home to assess your needs and discuss how best to meet them. This will include documenting the medical and mental conditions that you are experiencing. If your medical condition is not severe enough to require in-home support, your service provider may assign you to a primary care physician or a home health care nurse. You can receive individual home care which covers by your health insurance, Medicaid, or a State Supplement Insurance Plan (SSPP). SSPP pays for the full cost of in-home support.
